3. I authorize my current high school to furnish all academic and personal information requested by the Office of Admissions of K12 Prep Academy.
4. I authorize K12 Prep Academy to report my academic progress to my high school.
5. I understand that my enrollment will be limited to courses approved by the K12 Prep Academy advisors. Once the student has completed the enrollment forms and the student username/id has been issued, all enrollment fees are non-refundable. Payment Plans are acceptable. If you cancel a payment plan, you are still responsible for all tuition fees due to the date of withdrawal. A tuition fee is charged for each month of attendance. The student has 365 days to complete a school year. At the end of 365 days, an extension request can be made which will allow an additional 60 days access for an extension fee.
